Church Disaster Preparedness Survey

The Christ’s Church Disaster Preparedness Ministry does not replace emergency services provided by government agencies. However, our ministry is concerned about your welfare during times of disaster. We are also concerned about our community. This survey will assist the Church Disaster Preparedness Ministry Team to identify individuals that wish to volunteer their services. As a church body, we desire to be prepared to serve each other and our community in the event of an evacuation, emergency, or disaster.

Please direct questions about this form and the Disaster Preparedness Team to Josh Barnhart (276) 806-5936. Thank you for prayerfully participating in this survey!
